This will give us a chance to discuss your concerns and determine if Carolina Speech Works, LLC is good fit for your needs. These calls generally last about 15 minutes. You may text, call, or email us to set up this consultation. Customer information is never shared with third parties/affiliates for marketing/promotional purposes.
REFERRAL Before we begin therapy, a referral for speech therapy must be received from the client's primary care physician (if we are billing insurance). This can be faxed to our office (803.281.8877), or emailed ([email protected]). Once we receive the referral, we will verify your insurance benefits and schedule an evaluation. No referral is needed for the initial phone consultation.
PAPERWORK Once you are scheduled for an initial assessment, you will be sent a link to our practice portal to fill out some paperwork. We request that you log into our practice portal and complete the intake paperwork 48 hours prior to the evaluation so that we can focus all of our time on the assessment process during your appointment.
EVALUATION Once the physician's order for therapy is received, an initial evaluation will be scheduled. The evaluation will consist of formal and informal assessments. Evaluations generally last approximately 90 minutes. At the conclusion of the assessment, a review of the results along with recommendations regarding focus of treatment and frequency of therapy will be discussed. The client and family are encouraged to collaborate with the speech therapist in the formation of therapy goals, in order to ensure the therapy plan is functional, relevant, and client-centered. A written report detailing the assessment findings, recommendations, and treatment plan will be reviewed at the first treatment session following the evaluation, and can be sent to any other healthcare providers involved with the client's care as requested.
TREATMENT Once a plan of care has been established, the client will begin treatment sessions at an agreed upon frequency of weekly visits based on the client's diagnosis, the evaluation findings, the therapist's recommendations, the client's goals, and the client's availability. Regular progress notes will be completed and reviewed with the client in order to allow for adjustment of goals and optimal progression of treatment.
